Abstract

Sustaining quality in healthcare remains a complex and persistent challenge across health systems worldwide. This paper applies the MICE Matrix—Money, Ideology, Compromise, and Professional Ego—as an analytical framework to decode the underlying behavioral and systemic factors influencing the sustainability of healthcare quality. The framework moves beyond conventional structural or process-based assessments and examines how economic incentives, ideological orientations, institutional compromises, and professional egos collectively shape the culture of care and performance outcomes. The “Money” dimension captures the financial drivers and constraints that influence resource allocation and incentive structures. “Ideology” reflects the value systems and philosophical orientations—such as healthcare as a right versus a commodity—that affect organizational priorities. “Compromise” denotes the negotiated trade-offs among stakeholders balancing cost, access, and excellence, while “Professional Ego” represents hierarchical dynamics, resistance to change, and professional identity conflicts that hinder collaboration. Drawing on theoretical insights and illustrative examples from both public and private healthcare contexts, the paper contends that quality improvement must be viewed as a social and behavioral process embedded in institutional realities rather than a purely technical pursuit. The study concludes that addressing the four MICE dimensions holistically—through aligned incentives, shared ideology, transparent governance, and inter-professional respect—is essential to achieve sustainable, patient-centred quality in healthcare delivery. This conceptual framework model offers a novel tool for researchers and policymakers to design integrated strategies that bridge human behavior, institutional culture, and systemic reform.
